How admission works
- Referral. Most patients are referred from an acute-care hospital, but self-referrals and physician referrals are welcome.
- Clinical review. Our team reviews medical records to confirm the patient meets criteria for inpatient rehabilitation.
- Insurance verification. We work directly with payers to verify benefits and obtain authorization.
- Admission. A liaison meets the patient and family to coordinate transfer.
What to bring
- Photo ID and insurance cards
- Comfortable, loose-fitting clothing and closed-toe shoes
- Personal hygiene items
- Current medication list and assistive devices (glasses, hearing aids, dentures)
